Plan Effective Dates

  • Fall 12-month plan: August 1st – July 31st, 2027
  • Fall 6-month plan:  August 1st – January 31st, 2027
  • Spring 6-month plan: February 1st – July 31st, 2027
  • Note – Vision Insurance is always a 12-month plan with effective dates of August 1st – July 31st
  • Your MetLife Dental group number is: 277272
  • Your Member ID is your Grand Canyon University Student ID.
  • Please allow 10 business days for your insurance to be activated. 

How do I use my benefits?

MetLife Claims Process:

  • Please note that it is always recommended that your provider submit your claims. If you see an in-network or out of network provider and submit your claim yourself, it will always be covered as an out of network expense. 

MetLife Dental PPO Plan:

  • Once enrollment is activated, you can register with MyBenefits to print your ID card and view claims information. 
  • When creating a MyBenefits account, under “Access MyBenefits”, please enter “Grand Canyon University Student Plan“. When registering your account, you will be asked for a “MetLife Assigned ID”. This is your Grand Canyon University Student ID. If it is less than 9 digits, please add zeros to the beginning. 
  • Registration in MyBenefits is not necessary to utilize your benefits.  
  • If you do not have an ID card, your provider can verify your coverage using your name, date of birth, and Grand Canyon University Student ID.
  • To find a dental provider, please visit MetLife.com. Next to “How can we help you?” select “Find a Dentist” and choose “PDP Plus”. Enter your Zip, City or State and select “Find a Dentist”.
  • You will get the best benefit by going to an in-network provider. Out of network care is subject to a Maximum Allowable Charge.
  • The MetLife Mobile App is available on the App Store and Google Play. Once downloaded, it can be used to find a participating dentist, view claims and to see your ID card.  

MetLife Dental Customer Service:

Davis Vision by MetLife Plan:

  • Your MetLife Vision Plan uses the Davis Vision network. 
  • A vision ID card is not provided. Your vision provider will use your name and date of birth to verify coverage.
  • You may access information about your plan by creating an account on the MyBenefits website. 
  • When creating a MyBenefits account, under “Access MyBenefits”, please enter “Grand Canyon University Student Plan“. When registering your account, you will be asked for a “MetLife Assigned ID”. This is your Grand Canyon University Student ID. If it is less than 9 digits, please add zeros to the beginning.
  • To find a vision provider, please visit MetLife.com. Next to “How can we help you?” select “Find a Vision Provider” and choose “MetLife Vision – Davis”.

Davis Vision Customer Service:

  • For questions related to claims or benefits please contact the Davis Vision customer service team (833) 393-5433.

Teladoc Plan:

  • To begin using your benefits, download the Teladoc App. Search the app store for “Teladoc”.
  • Enter your basic information to confirm eligibility.
  • To set up your new account locate and tap the “set up your account” tab at the bottom of the login page.
  • Enter the requested information to locate your account.
  • Once prompted, select “HUB International” as your group affiliation.
  • Confirm and complete the next registration steps.
  • Establish a secure password and necessary security steps for log in purposes.
  • You will need to update your medical history upon completion of the account setup.
  • You can request consultations upon the completion of your medical history.
  • Your coverage will become effective on the first business day after you have enrolled.
